Annual Renewal of Membership Letter. (Self-explanatory) Please attach a copy of your ID. 2. Application for Membership A requirement of the Association is for members to complete this form annually to ensure that personal particulars are correct and updated. 3. Indemnity Form It is compulsory for all members to complete this form.
